Provides assistance with homelessness prevention and transition to permanent housing, outreach and case management with referrals to other resources. To qualify, must be homeless or at risk and have any discharge status other than dishonorable while serving at least 1 day of active duty.

Per diem short-term transitional housing and case management for veterans. Up to nine-month program for vets and their families. Emergency transitional housing for singles only up to 30 days with possible extention to 90 days maximum, VA must refer.
